Health & Required Documents
Provide the documentation Oklahoma childcare standards require.
- Current immunization record (or signed exemption)
- Most recent well-child physical from your pediatrician
- Copy of birth certificate and any custody documentation
- Allergy / medication action plans, if applicable

Enrollment Confirmation
We confirm your spot and finalize details together.
- Receive a written confirmation of your child’s start date
- Review and sign the parent handbook acknowledgment
- Set up authorized pickup and emergency consents
